Welcome to the Conyers SDA Church - Hope Food Pantry. This computerized record-keeping system, administered by the Atlanta Community Food Bank, captures information about individuals in need of emergency services. Rest assured that your information will be managed confidentially and securely. Personal information is not disclosed; only statistical data is shared. Please, ONLY ONE person per household should register. Provide your address and the names and dates of birth for everyone in your household. By doing this, you may qualify for other programs such as T.E.F.A.P. or G.N.A.P. (fill out the income/expense section). In addition, we are required to keep an accurate count of the number of persons we serve. Remember to register only ONCE. Be sure to include the name of the Proxy who will pick up on your behalf. If their name is not on your profile, we will not allow them to pick up for you. To ensure everyone gets food, we limit each household to ONE BOX of food each distribution day. Remember to input a valid email address & mobile number.
